Your advice is all good, but for reasons that i feel are not mine to go into, it's not easy to apply it. I can't begin to imagine NOT taking responsibility for breaking something, but i know that cultures of blame and shame can really make it hard to do so. Add in systems that insist on the financial bottom line as all that matters and ... well, i guess i can imagine. I admire your willingness to argue with bosses. I'm like that too. The world needs it.
